Whether you're using insurance or paying directly, we want costs and expectations to be clear before your visit.

Storm Wellness Northwest now accepts many Aetna and UnitedHealthcare plans for eligible medical care. Your out-of-pocket cost depends on your specific benefits, including copay, deductible, and coinsurance.

We're actively credentialing with additional insurance carriers and will update this page as new plans come online.
Using insurance
Here's what to expect.
When we bill your health plan, please bring your current insurance card and let us know right away if your coverage changes. A few things worth knowing:
- We're happy to verify benefits before your visit. Coverage and final patient responsibility are determined by your health plan when it processes your claim.
- You're responsible for understanding your plan's deductibles, copays, coinsurance, and any referral or prior-authorization requirements.
- Some services — like aesthetic treatments, certain hormone therapies, and some weight-management services — may not be covered by your plan even when we're in network. Coverage often depends on your diagnosis and your plan's medical-necessity criteria.
- If a service isn't covered, we'll tell you the cost before it's provided, whenever possible.
Self-pay
What care costs if you pay directly.
Insurance isn't required to receive care at Storm Wellness Northwest. For patients paying directly, here's what office visits cost — payment is due at the time of service.
New Patient Intake Visit

Follow-Up Visit

A focused visit to review progress, labs, and next steps.

- Many Medical Care visits can be billed to insurance instead.
- If you're uninsured or paying cash, federal law entitles you to a Good Faith Estimate of expected costs before your visit — just ask.
- HSA and FSA cards are accepted for qualified medical expenses.
- Aesthetic treatments are elective and self-pay. Pricing for neurotoxins, fillers, laser, and skin treatments is listed on each service page.
